Dual Relationship Consent Form (to be completed whenever a) an unrelated Adult Athlete shares a hotel room or sleeping arrangement with a minor athlete; b) an unrelated Applicable Adult provides local transportation to a minor athlete; c) a licensed massage therapist or other certified professional or health care provider treats a minor athlete as part of a team activity; d) an unrelated Applicable Adult to travels to a Stingrays team competition with a minor athlete; or e) any other exception to any area of the MAAPP is required due to a dual relationship existing between a minor athlete and a non-related Applicable Adult or Adult Athlete.)
